What is a Biological Indicator (BI)?

A Biological Indicator (BI) is a critical tool used in sterile processing to verify the effectiveness of sterilization processes. It contains highly resistant microorganisms—typically spores of Geobacillus stearothermophilus or Bacillus atrophaeus—that are used to challenge the sterilization cycle. If the sterilizer is functioning correctly, these organisms will be killed, confirming the process was successful.

BIs are considered the gold standard in sterilization monitoring because they directly assess whether microbial life can survive a given cycle. They are most commonly used to test steam, ethylene oxide (EtO), and other low-temperature sterilization systems.

Why are Biological Indicators Important?

Biological Indicators play an essential role in patient safety by confirming that surgical instruments are properly sterilized before use. While chemical indicators and mechanical data provide process information, only BIs offer biological proof of sterilization efficacy.

This is especially important in high-volume healthcare facilities, where hundreds of instruments are processed daily. A failed BI indicates a potentially dangerous sterilization failure that could lead to surgical site infections, regulatory noncompliance, or patient harm.

Regular BI use supports compliance with standards set by organizations such as AAMI, CDC, and The Joint Commission. Many hospitals also use automated BI readers to speed up results and improve documentation accuracy.
