What Is IUSS (Immediate-Use Steam Sterilization)?

Immediate-Use Steam Sterilization (IUSS) is a method used to quickly sterilize surgical instruments that are urgently needed and cannot go through the full sterilization cycle. Formerly known as “flash sterilization,” IUSS uses high-temperature steam under pressure for a shorter cycle and is performed when a critical item is required immediately, and no sterile replacement is available.

Hospitals use IUSS to minimize procedural delays, especially in high-turnover environments such as trauma units, labor and delivery, and operating rooms. However, this method must be used sparingly and under controlled conditions to maintain safety and regulatory compliance.


Why Is IUSS Important in Surgical Operations?

In urgent clinical situations, delays caused by unavailable sterile instruments can jeopardize patient safety and disrupt schedules. IUSS provides a fast solution, allowing staff to sterilize instruments and return them to service in a short timeframe—usually within minutes.

That said, IUSS is not intended for routine sterilization. Regulatory bodies like AAMI, CDC, and The Joint Commission emphasize that it should be reserved for emergencies or unplanned circumstances. Overreliance on IUSS often signals deeper issues, such as inventory shortages or inefficient reprocessing workflows.

To ensure compliance and patient safety, hospitals must follow strict protocols. This includes using sterilizers validated for IUSS, ensuring proper cleaning before sterilization, and documenting each cycle. Many facilities track IUSS usage with instrument tracking systems, integrated with EHR or ERP platforms for audit readiness and quality assurance.
